Swap the exercise, don't skip the muscle
Most people pick one of two bad options.
- Push through it — the pain grows and eventually costs weeks off entirely
- Cut the whole body part — drop everything for a cranky lower back and there is not much program left
There is a third option. Train the same muscle through a different joint angle. If back squats bother your lower back, leg press and hack squat hit the same quadriceps with a completely different spinal load.
This means finding an angle you can load without pain — not gritting through a movement that hurts. Pain lasting more than three weeks, or anything with numbness or tingling, is a doctor's job before it is a programming job.
The swaps are predictable by area
- Lower back — move off axial loading (back squat, deadlift, good morning) toward machines. A back pad is the marker
- Wrist — move from a fixed pronated grip (straight bar) to neutral (dumbbell, EZ bar, rope)
- Shoulder — avoid angles that drive the arm behind the torso (bottom of a barbell bench, behind-the-neck press) and keep range in front of the body
- Knee — move from knee-dominant work (lunges, sissy squat) toward hip-dominant (hip hinge, leg curl)
- Ankle — the problem is the heel lifting. Use a heeled lifting shoe, or move standing work to a seated version
